The link at the bottom of this page will take you to an application form. This form can be filled out online and returned to the parish office when completed. Be patient—it might be a few seconds after the form loads before you can start filling in the blanks.

If you are filling out the form online, we recommend using Internet Explorer as your browser.  If you are using Firefox, select Open With Different Viewer in the upper right, then select Adobe Acrobat Reader. Do not use Google Chrome or Opera; they will allow you to enter data, but will not save your entries. 

To complete the application process:

1. Please fill in all the blanks on the form. If you will be emailing the form, save it to a convenient location on your device and send it to [email protected].
2. If you prefer not to email the form, please print it and:
   • Place it in the collection basket
   • Deliver it to the parish office
   • Send it via postal mail to the parish office
